  • One set across independent licenseesEach licensee publishes its own index. The figures describe a single loaded file set assembled from those indexes, not one publisher’s disclosure.
  • Highmark is largely absent for JulyThe download links in the six Highmark plans' July indexes expired before their files could be retrieved, so those plans are mostly missing from this set.
  • In-network rates onlyAllowed-amount file sets are published separately and are not loaded.
